Process to be a registered patient:
  • If you are a New York state resident suffering from one of the following conditions cancer, HIV/AIDS, ALS, MS, spinal cord injury with spasticity, Parkinson's disease, Huntington's disease, epilepsy, IBD, neuropathies, chronic pain you may be eligible to receive medical marijuana. The process begins with your healthcare provider. If your provider is not registered with the New York State Medical Marijuana Program, he/she can recommend a registered provider by accessing the Medical Marijuana Data Management System in the New York State Health Commerce System at You may also find a list of practitioners who have consented to be listed publicly on the Medical Marijuana Program webpage at

    Once you have been certified as a medical marijuana patient by a registered provider you will need to create an account through the website. Follow the pathway through the "Health Applications" tab and "Medical Marijuana Data Management System" and once you have completed the application process and you are approved you should receive your card in a few business days.

    Please be sure to bring your medical marijuana card, a New York state issued photo identification, your written certification form from your provider and form of payment. Please call ahead and inquire about the forms of payments currently accepted.

  • What conditions make me eligible to obtain medical marijuana?
    You may eligible if you have been diagnosed with cancer, HIV/AIDS, ALS, Parkinson's disease, multiple sclerosis, spinal cord injury with spasticity, epilepsy, inflammatory bowel disease, neuropathy, Huntington's disease with associated or complicating conditions of cachexia or wasting syndrome, severe or chronic pain, severe nausea, seizures, or severe or persistent muscle spasms.

  • What is the first step to obtaining medical marijuana?
    The first step is speaking with your healthcare provider about medical marijuana and if it could be beneficial in your treatment. If your provider is registered with the New York State Department of Health's Medical Marijuana Program, he or she may issue you a certification for medical marijuana.
